John M. Miguelez, CP, FAAOP
Founder and President

John Miguelez founded Advanced Arm Dynamics in 1998 with the recognition that every orthotic and prosthetic facility wants to provide the highest level of care for its patients and wants to thrive in the increasingly competitive health care market. He has specialized in upper-extremity prosthetics for 18 years.

Mr. Miguelez is the former Founder, Vice President and Senior Clinical Director for the National Upper Extremity Prosthetic Program at NovaCare, Inc., now Hanger Prosthetics & Orthotics. After launching NovaCare’s Program in 1991, Mr. Miguelez successfully led 28 regional upper-extremity prosthetic specialists, the Upper Extremity Research and Fabrication Center and a national team responsible for patient coordination and insurance justification serving 3,000 patients a year.

Mr. Miguelez has published several articles in the Journal of Prosthetics and Orthotics and the Atlas of Amputations and Limb Deficiencies and conducts lectures nationally and internationally. He manages clinics throughout the United States for patients with challenging upper-extremity prosthetic requirements. Mr. Miguelez is also involved in several research and development initiatives, working closely with upper-extremity component manufacturers on education, component refinement and design.

Since 2003, Mr. Miguelez has led a team of senior clinicians at Walter Reed Army Medical Center in Washington, D.C., providing care at the request of the Department of Defense to injured U.S. soldiers from Iraq and Afghanistan. At Walter Reed Army Medical Center, Mr. Miguelez and his team offer prosthetic services to patients with amputations of all levels and in all areas of the rehabilitation process.

Mr. Miguelez attended the University of Southern California and received his Prosthetics Certificate from Northwestern University. He is an American Board for Certification (ABC) certified prosthetist and is certified by all leading component manufacturers for upper-extremity prosthetics, including Otto Bock, Motion Control, Liberating Technologies and Hosmer. He has served as a clinical instructor for Motion Control and Liberating Technologies for several years. Mr. Miguelez is a Fellow of the American Academy of Orthotists and Prosthetists and the former Chair of the Academy’s Upper-Limb Prosthetics Society. He is also a member of the board of directors for the International Society of Prosthetists and Orthotics (ISPO).

Chris Lake, CPO, FAAOP
Southwest Clinical Director     

Chris Lake is a native Texan and graduated in 1995 from the University of Texas Southwestern Medical Center Prosthetics and Orthotics program. He graduated from the Prosthetic Residency at Southern Illinois University and the Orthotic Residency at the University of Oklahoma. Before joining Advanced Arm Dynamics in 2000, he worked with Upper and Lower Extremity patients at Rehab Designs of America.

Mr. Lake is the Southwest Clinical Director for Advanced Arm Dynamics’ Center of Excellence located in Dallas, Texas. As an ABC certified and Texas licensed prosthetist-orthotist, he coordinated the construction of Advanced Arm Dynamics’ Center of Excellence in Dallas. Mr. Lake is a member of the American Academy of Orthotists and Prosthetists (AAOP), AAOP Upper-extremity Society, International Society of Prosthetists and Orthotists and is a fellow of AAOP.

Mr. Lake’s active involvement in industry research and development demonstrates his commitment to the advancement of upper-extremity prosthetics. He directs the Southwest Center as a testing facility for beta components for leading upper-extremity manufacturers. He conducts lectures all over the world and has published articles in the Journal of Prosthetics and Orthotics, Medical Engineering and Physics and The Atlas of Amputations and Limb Deficiencies. He also leads a team of Texas-based engineers in the development of new and customized upper-extremity components to meet the ever-changing needs and challenges of those with upper-extremity deficiency. In addition to his on-going accomplishments, he has recently been appointed as a Clinical Instructor at University of Texas Southwestern Medical Center.

Dan Conyers, CPO
National Clinical Director

Dan Conyers, certified prosthetist and orthotist, is the Northwest Regional Specialist for Advanced Arm Dynamics. He has over 19 years of experience in prosthetics and orthotics specializing the past ten years in upper-extremity prosthetics. He is ABC certified in orthotics and prosthetics and a member of the American Academy of Orthotics and Prosthetics (AAOP) and AAOP Upper-extremity Society.

Mr. Conyers resides in Portland, Oregon, where he manages the care of upper-extremity patients in the Northwest. Over the past several years, he has worked with practitioners and their patients in their local prosthetic offices on behalf of Advanced Arm Dynamics. In addition to these consultation services, he has provided prosthetic services and rehabilitation to patients at Walter Reed Army Medical Center in Washington, D.C., working with amputees of all levels.

MacJulian Lang, CPO
Northwest Clinical Director

MacJulian (“Mac”) Lang is the Clinical Director of Advanced Arm Dynamics of the Northwest which opened in April 2008.  Prior to leading the team in Portland, he spent two years at Walter Reed Army Medical Center in Washington, D.C. where he was a member of the Advanced Arm Dynamics team providing upper extremity prosthetic services to injured service members.

Mr. Lang has over ten years of experience providing upper extremity prosthetic care. After receiving a Bachelor of Science degree in mechanical and aerospace engineering from Cornell University, he put his engineering background to use in a field where he could interact directly with those his work would affect. Mr. Lang attended the orthotics and prosthetics program at California State University Dominguez Hills and completed his residencies in Portland, receiving his ABC Certification in prosthetics and orthotics.  His passion for providing the best prosthetic care available led him to join AAD in 2006.

He is a member of the American Academy of Orthotics and Prosthetics (AAOP), AAOP Upper-extremity Society, and International Society of Prosthetics and Orthotics and continues to contribute to the prosthetics field by presenting research and prosthetic techniques.  He has been published in the Journal of Prosthetics and Orthotics, along with co-authoring a chapter on Upper Extremity Prosthetic Care in the “Care of the Combat Amputee” medical textbook.

Patrick Prigge, CP
Upper Extremity Specialist

PatrickPatrick Prigge, CP joins AAD as an upper extremity specialist focusing his efforts as the clinical upper extremity specialist at AAD of the Midwest. In addition, he will also spend time with the Walter Reed Army Medical Center (WRAMC) team based in Washington, DC providing care to American military personnel who have suffered the loss of a limb in combat.

Pat worked previously as the Director of Prosthetics Education for Otto Bock HealthCare in Minneapolis, Minnesota. His responsibilities included developing a team of prosthetists and occupational therapists dedicated to bridging the gap through education between fitting highly advanced new technologies by the prosthetist and full acceptance and usage by the end user. He also had responsibility for education content and strategy and was involved with product development. In addition to his time with Otto Bock HealthCare, Pat’s work experience includes working as a clinical prosthetist for both Hanger and Gillette Children’s Hospital.

He received his post graduate certificate as a prosthetics technician and practitioner from Century College and earned his Bachelor of Arts (BA) degree in Biomedical Science from St. Cloud State University.

Craig Jackman, CPO, FAAOP
Upper Extremity Specialist

Craig Jackman joined the Advanced Arm Dynamics team in late 2007.  Craig is an upper extremity specialist focusing his time with the Walter Reed Army Medical Center (WRAMC) team based in Washington, DC providing care to American soldiers who have suffered the loss of a limb in combat. Previously, Jackman worked as an upper extremity specialist for Hanger Orthotics and Prosthetics in Columbus, Ohio. His responsibilities included supporting the Ohio State University Prosthetics and Orthotics clinic and the MDA clinic at OSU. He received his certificate in prosthetics and orthotics from the Newington Certificate program and earned his Bachelor of Science (BS) degree from Rutgers University.

Ryan Spill, CP
Upper Extremity Specialist

Ryan Spill joins the Advanced Arm Dynamics team as an upper extremity specialist. His responsibilities include working with the Walter Reed Army Medical Center team providing upper extremity care to soldiers wounded in combat. Spill’s previous experience included responsibilities as a clinical representative for Motion Control,Inc. His experience includes training prosthetists worldwide in external powered fitting techniques. He received his prosthetic certificate from California State University, Dominguez Hills and his Bachelor of Science degree in health sciences from Lock Haven University.   

John Costello, CPO
Midwest Upper Extremity Specialist

John Costello is the Midwest Upper Extremity Specialist for Advanced Arm Dynamics.  ABC certified in both orthotics and prosthetics, Mr. Costello has worked in the prosthetic and orthotic field for over 12 years, bringing a wealth of knowledge to Advanced Arm Dynamics.  Mr. Costello is also a partner at Clark and Associates, a provider of prosthetic and orthotic services.

After graduating with a bachelor of arts degree from Coe College in 1992, Mr. Costello went on to earn his certificate of orthotics in 1994 and his certificate of prosthetics in 2000, both from Northwestern University.  He began his career in 1994 with Dale Clark Prosthetics.

Mr. Costello’s philosophy is to be positive and open minded while achieving the highest level of patient care by truly listening to his patients.  Heand his wifecurrently reside in Dike, Iowa.  They have one daughter, Ella Rae

Kristin Gulick, OTR/L, CHT
Director of Occupational Therapy Services 

Kristin

Kristin Gulick is the Director of Occupational Therapy Services for Advanced Arm Dynamics. She has more than 19 years of experience in occupational therapy focusing the past ten years on upper-extremity rehabilitation. After earning a Bachelor of Science in occupational therapy from the University of Puget Sound in 1985, she began her career at Shriners Hospital focusing on children with congenital limb loss and limb anomalies. Her work in pediatrics led her to complete certification in neurodevelopmental therapy. Ms. Gulick later served as the clinic coordinator for the outpatient hand therapy clinic at Tampa General Hospital in Florida, a level one trauma center. During this time, she earned her Certified Hand Therapist credentials. In 1998, she returned to Portland, Oregon where she worked for a private hand therapy practice before joining Advanced Arm Dynamics.

Ms. Gulick provides pre-prosthetic evaluation and training, prosthetic training and post-prosthetic follow-up to clients receiving services from Advanced Arm Dynamics. Her teaching and interpersonal skills provide patients nationwide with the training that allows them to gain full benefit from their prosthesis. Ms. Gulick also trains therapists in patients’ communities to establish local resources. She frequently shares her knowledge formally at state and national professional meetings and is involved with industry-related research and development.

Kristi Wolfgram
National Patient Coordinator  

Kristi Wolfgram is the National Patient Coordinator at Advanced Arm Dynamics. She serves as a liaison between the patients, reimbursement agencies and Advanced Arm Dynamics' upper-extremity prosthetic specialists to ensure that each patient's goals and concerns are carefully addressed.

Ms. Wolfgram, who has a congenital below the elbow deficiency, offers patients and their families empathy and guidance for an independent future. Born and raised in Omaha, Nebraska, she received her first prosthesis at six months of age. Since then, body-powered and electrically-powered prostheses have helped her enjoy an extraordinarily accomplished and active life.

Ms. Wolfgram earned a degree in broadcasting from Northwest Missouri State University where she was a member of Alpha Sigma Alpha Sorority and the university’s nationally ranked dance team, The Steppers. She then joined the dance team for The Racers, Omaha's professional basketball team and later joined the dance team for The Omaha Beef, Omaha's national indoor football league. Ms. Wolfgram also teaches dance lessons in tap, jazz, ballet, pom pom and hip hop at a local dance school. As an adolescent, Ms. Wolfgram enjoyed swimming, ice-skating, gymnastics, softball and horseback riding. As an adult she continues activities in dance and water-skiing.
